2. On the left hand side of the form, list the criteria relevant to the application. Remember there are six different types of applications, each with a different set of criteria. (Most departments have only two of the three levels listed.)

(a) level 0, initial appointment

(b) level 0, reappointment

(c) level 1, initial appointment

(d) level 1, reappointment

(e) level 2, initial appointment

(f) level 2, reappointment

3. The criteria for your type of application can be copied from your department’s plan.

4. On the right side of the form, list quantity, if relevant, and vita page number for each criterion listed on the left. Please be concise. For instance, if the criterion is “must have three full length papers published in refereed journals,” one might say on the right, “5 papers satisfy this requirement; see vita page 24.” Make sure the vita lists dates for the papers. Only papers published during the last 5 or 7 years are counted depending on your departmental plan.

5. If your departmental criteria require senior authorship, please indicate in your vita the publications for which you are the senior author.

6. If graduate committee work is a requirement, you may need to supplement your vita with the names of the graduate students and the dates of their graduations, as well as, for each committee, whether you were chair of the committee or a member of the committee.

7. Departmental votes are usually required only of level 2 applicants. Please check your departmental plan.

8. Mail your completed application cover form and vita to the Graduate School, Hargis Hall.
