Graduate Student Health Insurance Program
WAIVER REQUESTS
The waiver request form is for graduate students who are included in the mandatory group.
GA/GTA/GRA students who fall under the mandatory coverage requirement may waive out of this policy only by providing evidence of coverage by a policy that is equal to or greater in coverage than the entirety of the AU policy. All waiver requests must be received by the
15th class day of either the Fall or Spring semesters. Students who waive out of the GSGHP/ISSHP will NOT be eligible for the university subsidy.
International graduate students in F or J immigration status are subject to the conditions of the Mandatory International Student and Scholar health insurance plan, check with OIE.
All waiver requests must be received no later than the 15th day of classes

Print the appropriate waiver form, complete it according to the
instructions, and return to the Graduate School. You must also
submit a copy of the front AND back of your current insurance card
with the waiver form.
