Graduate Student Health Insurance Program
WAIVER REQUESTS
The waiver request form is for graduate students who are included in the mandatory group.
GA/GTA/GRA students who fall under the mandatory coverage requirement may waive out of this policy only by providing evidence of coverage by a policy that is equal to or greater in coverage than the entirety of the AU policy. This requirement must be demonstrated by completion of the waiver form by the proposed insurance company or an official representative of the insurance company (HR benefits officer), etc. All waiver requests must be received by the 9th class day of either the Fall or Spring semesters. Students who waive out of the GSGHP/ISSHP will NOT be eligible for the university subsidy.
International graduate students in F or J immigration status are subject to the conditions of the Mandatory International Student and Scholar health insurance plan, check with OIE.
ALL WAIVER REQUESTS MUST BE RECEIVED NO LATER THAN THE NINTH DAY OF CLASSES.
Print the appropriate waiver form, complete it according to the instructions, and return to the Graduate School. Faxes will be accepted for preliminary review. However, original signed forms must be received by the deadline indicated on the form.
